Please use this form to report single, all-day, absence only.

Do NOT use this form for:

• Multiple Day Absences (phone call to 714-734-2900, Press #1)

• Partial Day Absences (phone call to 714-734-2900, Press #1)

• Early Dismissals (send note with student in the morning, or come in person to sign student out).

• Late Arrivals (send note with the student to submit upon arrival).

NOTE: Parents have 1 week to clear an absence.

Parents do NOT need to call in if this form has been submitted.
